Want your test results?

It's your right to view your own life insurance test results.

You can choose from two options. First, you may contact the insurance company directly. Ask them to send you an authorization form. Complete and sign the form, and send it back to them. Once they receive your completed and signed authorization, they'll send your test results directly to you.

Not everyone enjoys dealing with insurance companies, however. Therefore, your second option is to give us a call. We're happy to handle the insurance company for you. We'll obtain their authorization form, complete it with your case information, and send it to you. You simply sign it and return it to us. We'll forward the form to the proper department at the insurance company.

Either way, the insurance company will send your test results directly to YOU, not to us.
